When you submit the form, we collect your full name, date of birth, age, phone number, email address, home address, homeowner or renter status, preferred language and your consent to be contacted.
Why we collect it
Information submitted is intended to help the Participating Firms identify and respond to your inquiry, review potential legal matters associated with the June 17, 2026 warehouse fire, and communicate about possible next steps.
Who may receive it
Information may be accessed by the Participating Firms and personnel or service providers involved in processing, storing and reviewing requests. The website hosting and form processing providers also process information as needed to operate the form.
How to protect your information
Do not include Social Security numbers, medical records, financial account credentials, or other documents in this form. Internet submissions carry inherent risks, even when reasonable precautions are used.
